Request for a service provider to provide medical service in line with the following
requirements:

1. 10x Year1 – 2022 Retinography/Scan or picture of the back of the Retina of the eye
2. 10x Year2 – 2023 Retinography/Scan or picture of the back of the Retina of the eye
3. 10x Year3 – 2024 Retinography/Scan or picture of the back of the Retina of the eye

N.B. Retinography test to be conducted at supplier premises
