Request for a service provider to provide medical service in line with the following requirements:

• 6 x Conduct Hepatitis B immunity assessment for employees working with effluents
• 6 x Provided that employees DO NOT have immunity, a service provider to issue required immunity medication.

N.B. Hepatitis B Assessment to be conducted at supplier premises

• Assessment and required medication, where required, must be administered at suppliers medical facility.
